Obituary Order Form

All obituary/death notice orders from the San Jose Mercury and the San Francisco Chronicle must be pre-paid. The quickest and most convenient way for you to get your obituary is for you to fill out this online form, which will allow you to pre-pay using your Visa or MasterCard through PayPal. We will either send you the document as an Adobe Acrobat attachment or else notify you that we couldn't find it. The fee is for the search, not for the obituary, because the time and expertise are the same, whether there is an obituary or not. Please allow four weeks for your document or the notification that we couldn't find it.

Otherwise, you may contact us using the information on our order form. If you order by U.S. mail or fax, please print out and use this form

Please verify the name and date of death in the California Death Records (1940-1997) and the Social Security Death Index. Sometimes the record will be in only one of these.
